As a community club player / parent, we have to agree yearly to sign up to the clubs code of conduct.
This was posted out on all parents chat on Wednesday 12th Feb. 2025.
If not completed the form yet, please ensure this is completed ASAP. We only need one form per player returned as parent/guardians can be completed on thr same form.
We would appreciate a quick return of the form if not completed already.
